 What date are the W-2 forms legally required to be mailed out?

  • They must be postmarked by January 31, 2012.

 What address do you have on file for me?

  • The last address provided to InfoSync on or before December 20, 2011.
 

 How long will it take to receive my W-2?

  • The W-2 tax statements will be mailed by January 31, 2012. Please allow at least 10 days for delivery.
  • W-2 statements for active employees will be mailed directly to the restaurant as one store bundle.
  • W-2 statements for inactive employees will be mailed regular first class mail to the last known address on file.
 

 Can you fax or email a copy of my W-2?

  • W-2 statements can be faxed but NOT until the week of February 13th, 2012.
  • They cannot be emailed.
 

 What do I need to do in order to receive a reprint of my W-2 tax statement?

  • Reprints will not be available until the week of February 13th, 2012.
  • In order to request a reprint, call directly into the W-2 line at Toll Free (855) 685-1622 or (316) 681-8599.
  • Your call will be returned within 24 hours. Please leave the following information:
    • Complete Name.
    • Complete phone number including the area code.
    • Last 4 digits of your social security number.
    • Company name and state you are/were employed with.
    • Please speak slowly and distinctly.
  • There is no fee for a reprint.
 

 How do I change my address to receive my W-2 for 2011?

  • If you wish to change your permanent address in the Payroll System, you must fax a written request to 316.681.4328.
  • If you are an active employee contact your General Manager, complete a new Statement W-4 and have them send it to InfoSync.
  • Inactive employees must fax a written request including their name, social security number, old mailing address, and new mailing address to Payroll at 316.681.4328.
 

 Will a request for duplicate W-2 form change my address in the Payroll System?

  • No. The address specified on the request for Duplicate W-2 statement is only for the one-time mailing of the requested statement. Please send an address change to the Payroll Department for any changes pertaining to your future W-2 tax statements.
 

 Who is responsible for updating my address?

  • Every employee is responsible for updating his/her address as soon as an address change is known. In addition, the employee is responsible for reporting any changes to his/her status such as marriage, divorce, death or employment changes within 30 days to the Payroll Department.
  • Certain events may qualify the employee to make changes to his/her insurance elections if processed within 30 days.
  • Name changes require a new Social Security Card before the employees' name can be altered. The Internal Revenue Service may impose fines on employers if the employees name reported on the W-2 statement and pay records do not match the Social Security Administrations records.
 

 My son/daughter is in the military and needs a copy of his/her W-2. How do I get it?

  • Follow the instructions above and include a copy of an authorization form from the employee.
  • W-2 statements cannot be released to any other party without written authorization from the active or inactive employee.
 

 I don't understand what all the items are on my W-2.

  • See "Notice to Employee" on the reverse of copy B, and "Instructions" on the reverse of copy C.
 

 Why are my federal and state taxable wages different from Social Security wages?

  • Certain pre-tax deductions such as Section 125 plans including medical, dental, vision, life, flex medical and dependent care reduce federal, state, and Social Security wages. Other pre-tax deductions including NC state retirement, annuities, and 401K reduce only federal and state taxable wages.
 

 The amount listed on my W-2 is not the same as year-to-date earnings on my December pay stub?

  • The year-to-date earnings on your check stub are the actual amounts that you earned during the calendar year. Not all of your earnings are federal taxable; some of your deductions such as medical insurance are made before taxes. This reduces your taxable income. The amount on your W-2 statement is federal taxable earnings.
